Becoming a Patient

Step 1: Referral

It all begins with a referral. Valley Perinatal Services is a co-management practice and requires a referral in order to schedule you an appointment. Do you have a referral form filled out by your provider to us? Then you’re ready to go to step 2! If you don’t and you’re interested in our services, reach out to us here.

Step 2: Scheduling

Once a referral is received from your primary provider, you can contact us to schedule your appointment. It’s easiest if you just text us here:

Step 3: Getting Ready for Your Appointment

You’ll want to make sure you have the following ready and with you to bring to your appointment:

  • Health Insurance Card (if applicable)
  • Drivers License
  • To speed up the check-in process, once your appointment is scheduled you may create your patient portal account and pre-fill out your registration paperwork.

New Patient Information

You can easily access your health information through our secure Patient Portal. As soon as you book an appointment with us, your portal will link to your patient information.
